Crisis Ready Program
Do you worry what will happen if your loved one has a sudden and serious health problem? With the Crisis Ready Program, you and your family won’t have to scramble when unexpected hospitalizations or other crises arise. And if you’re a long-distance caregiver, you’ll have peace of mind knowing your loved one is in good hands, no matter what the crisis is. Here’s how the program works:
- It starts with an assessment of your loved one by a geriatric nurse, who gathers extensive medical information so she can act quickly when a problem arises. She’ll keep in contact with you and your loved one on a regular basis and answer any questions or concerns you might have. It’s like having a family member on-site with a medical background, who can provide the right assistance when it’s needed.
- When a crisis happens, we’re your eyes and ears on the scene, managing the details to make sure your loved one is getting the right care and that the physicians have the information they need.
- You’ll be kept informed on what home care services are needed to keep your loved one safe and comfortable. You can rest easy knowing your loved one is getting all the appropriate help to get through the crisis.
